Associate Continuing Healthcare Practitioner

An Associate Continuing Healthcare Practitioner Assessment Organisation is an approved company that is responsible for providing assessments for apprenticeships in the field of Adult Care. As an assessment organisation in the Adult Care sector, we will ensure that apprentices meet the required standards and competencies outlined in the relevant apprenticeship standards.

What the Role Entails

The Associate Continuing Healthcare Practitioner plays a key role in the assessment of individual’s complex health and social care needs that have arisen as a result of disability, accident or illness, and the planning and commissioning of the services for these individuals. They also act as the first point of contact for individuals and their carers offering support, care and advice about the service and for young adults and their family who are transitioning from children’s continuing care into this adult continuing healthcare service. They will also be required to have a detailed knowledge of the health and social care services available for these individuals who have complex and varying needs

Order of Assessment Methods

The assessment methods can be delivered in any order. The result of one assessment method does not need to be known before starting the next.

Performance in the Assessments determines the overall grade of:
• Fail
• Pass
• Distinction

Professional discussion underpinned by portfolio

This assessment will take the form of a professional discussion, which will be appropriately structured to draw out the best of the apprentice’s competence and excellence and cover the KSBs assigned to this assessment method. Steadfast Assessment will select questions from a standardised question bank and adapt these questions based on their review of the apprentice’s portfolio of evidence.

Work based observation and questioning

Apprentices must be observed by an independent assessor completing work in their normal workplace, in which they will demonstrate the KSBs assigned to this assessment method. The observation may be split into discrete sections held over a maximum of 1 working day. In advance of the observation, apprentices will be provided with information on the format of the observation, including timescales. The employer and apprentice will need to gain appropriate client consent in advance of the observation process. Questions will be asked after the observation and the independent assessor will ask a minimum of 5 questions.
